Many of you have witnessed the rapid growth of medical spas over the past few years. All too often, these businesses are not affiliated with a physician whose training or expertise involves a medical specialty in which aesthetics is a key component. A patient undergoes a laser treatment for hair removal and develops a skin complication. The physician directing care is an obstetrician/gynecologist, and is not considered an expert or resource for such complications. What does the patient do, and where does the patient seek help? A patient wants facial wrinkle removal, and sees an advertisement for a “no-down time” laser procedure. She pays $ 4,000 for four sessions and six months later does not see any appreciable change. A patient underwent one Botox cosmetic treatment in the past but was dissatisfied. Was she treated optimally, or was the dosing choice poor? Will she be less likely to seek Botox treatment again because her expectations were not met? Was the original treatment a case of under-dosing and overcharging?
A patient is considering facial fillers to improve her facial folds and wrinkles. She reads about Restylane, Perlane, Juvederm, Radiesse, and Sculptra, and becomes quite confused. How well will she be guided toward the right filler for her situation, and how skilled is the staff at administering these various fillers?
A patient wants to have her skin tightened to look younger. She has excessive skin laxity to achieve good results with lasers and non-surgical options, but she seeks care at a medical “spa” that is not directed by a plastic surgeon. Will she be treated with a non-surgical therapy anyway and be disappointed? Will she receive a burn to the skin as the energy settings are set extremely high because the practitioner is trying to achieve changes that really require a facelift or eyelid procedure?
A medical “spa” wants to offer laser hair removal and discovers that one of their existing lasers can be used for that purpose, but it is not a laser that “specializes” exclusively in hair removal. Will the patient achieve effective results after undergoing a treatment series, or will her treatments be ineffective requiring her to seek more treatment sessions or change to another medical “spa?”
A patient is seeking reduction in wrinkles around the mouth (“lipstick lines”). She is not given a deep anesthetic block, and experiences considerable discomfort as the treatment is given. Could that patient have been a lot more comfortable during her treatment?
